Open Admissions Policy

Annunciation School has an open admission policy. No person, on the grounds of race, color, disability, or national origin, is excluded or otherwise subjected to discrimination in the receiving of services.

Application Process

Please contact Mrs. Caprice in the main school office at 954-989-8287 to schedule a school tour, complete an online application form, and schedule a testing appointment.

On the day of the student’s testing appointment, please submit the following:

• Copy of the last quarter report card for the current year

• Copy of the final report card for the last year at the current school

• Copy of the standardized test results (TerraNova, FSA, or equivalent) for the last two years (if applicable)

•  A completed Florida Certificate of Immunization (Form DH 680) is required to document the administration of prescribed immunization doses. An exemption from immunization requirements is permissible only with a physician’s certification as to the need for either a temporary or permanent medical exemption. An exemption from immunization requirements is not permissible for religious, philosophical, personal, or other reasons.

After careful consideration of the application and testing results, an interview may be granted. In matters of admission and registration, the decisions of the Pastor and Principal are final.

Registration Process

Once a student has been accepted, the following must be provided to complete the registration process:

• Completed Registration Form – provided by office

• Copy of Birth Certificate

• Copy of Baptismal Certificate (if applicable)

• Copy of First Holy Communion Certificate (if applicable)

• Health Forms (due before the first day of school)

• Non-refundable Registration Fee
